Helicopter crash: Crew had flown three flights before the accident – Investigator

An aviation investigation expert, Captain Paul Forjoe, has disclosed that the crew involved in the August 6 military helicopter crash had completed three flights prior to the tragic incident.

Speaking at a press conference in Accra on Tuesday, November 11, to release details of the investigative report, Captain Forjoe confirmed that the crew had observed a 24-hour rest period before the ill-fated flight.

“The last three flights before the accident were conducted by the same crew,” he emphasized.

He further noted that investigators conducted a thorough review of the crew’s qualifications, medical records, and psychological assessments as part of the inquiry.

The report was presented following Monday’s meeting at Jubilee House, where Acting Minister for Defence, Dr. Cassiel Ato Forson, led the investigative committee in submitting its findings to President Nana Addo Dankwa Akufo-Addo.

The crash involved a Harbin Z-9EH military helicopter (tail number GHF 631) operated by the Ghana Air Force. The aircraft departed Accra around 9:12 a.m. on Wednesday, August 6, 2025, en route to Obuasi in the Ashanti Region on an anti-illegal-mining operation before losing radar contact and crashing in the Adansi Akrofuom District.

All eight personnel on board tragically lost their lives in the crash.
